Brazilian Butt Lift (BBL)

The Brazilian butt lift (BBL) is an aesthetic surgical procedure performed by transferring fat tissue taken from the person’s own body to the hip and buttock area. Known as the Brazilian Butt Lift, this method aims to add volume to the buttocks and make the body contours more balanced.

In BBL surgery, fat tissue from areas such as the abdomen, waist, back or thighs is usually harvested by liposuction. The fat obtained is injected into the buttock area after being processed appropriately. In this way, the aim is to achieve both slimming in the areas where fat is removed and fullness in the buttocks.

Because the person’s own fat tissue is used in the procedure, no silicone implant is required. However, BBL is a surgical procedure with serious complication risks. Because of the risk of fat embolism in particular, it is important that the surgery is performed by an experienced plastic surgeon using techniques that comply with current safety standards.

What Is BBL and What Are Its Advantages?

The Brazilian butt lift is performed to increase buttock volume and reshape the waist-to-hip ratio. It can be considered especially in people who lack volume in the buttock area or who want more defined body contours.

One of the main advantages of BBL surgery is that fat tissue from different areas of the body is used. Taking fat from around the abdomen and waist can contribute to more defined contours in these areas, while the transferred fat can add volume to the buttocks.

Using natural tissue is one of the important features of BBL. Because the person’s own fat cells are used, no foreign filler materials are needed.

Shaping the body contours is another advantage of the procedure. By planning the areas where fat is removed and transferred together, the transition between the waist and hips can be made more pronounced.

Long-lasting results are among the advantages that a fat transfer performed appropriately can provide. While some of the transferred fat is absorbed by the body, the cells that stay viable can provide volume for many years.

People who are satisfied with their appearance after surgery may experience increased self-confidence. However, aesthetic and psychological results vary from person to person.

How Is the Brazilian Butt Lift (BBL) Performed?

BBL surgery is planned after an assessment of the patient’s general health and body anatomy. During the examination, the areas from which fat can be harvested, the current buttock volume, skin elasticity and the patient’s expectations are examined.

The procedure basically consists of three stages.

Harvesting the Fat Tissue

In the first stage, fat tissue is harvested by liposuction from areas such as the abdomen, waist, back or thighs.

The surgeon removes fat tissue from the designated areas in a controlled way through thin cannulas. The amount of fat to be harvested is determined according to the patient’s body structure and the planned volume increase.

Preparing the Fat Tissue

The harvested fat tissue goes through special processing to make it suitable for transfer.

At this stage, excess fluid, blood and unwanted components can be separated. The aim is to ensure that viable fat cells are transferred under appropriate conditions.

Transferring the Fat to the Buttock Area

The prepared fat tissue is transferred to the buttock area with the help of special cannulas.

One of the most important safety rules in BBL surgery is that the fat is injected only into the fat tissue under the skin. Injecting fat into or beneath the muscle can increase the risk of fatal fat embolism.

In current safety approaches, tracking the position of the injection cannula with real-time ultrasound is recommended. In this way, the aim is to place the fat in the correct anatomical layer.

The surgery is usually performed under general anesthesia and can take several hours. The duration of the operation varies depending on the number of areas to be treated with liposuction and the amount of fat to be transferred.

Preparation Before BBL

The patient’s general health should be evaluated in detail before BBL surgery.

During the examination, risk factors such as chronic diseases, regularly used medications, previous surgeries, smoking and clotting disorders are examined.

Smoking can negatively affect tissue blood supply and wound healing. For this reason, avoiding smoking before and after surgery is recommended.

Medications that may increase the risk of bleeding may need to be adjusted. However, prescription medications should not be stopped without the doctor’s recommendation.

The patient’s expectations are also evaluated before surgery. The amount of fat that can be transferred depends on the existing fat tissue and safe surgical limits. It may not be possible to achieve the desired volume with a single surgery in every patient.

Who Is Suitable for BBL?

BBL can be considered in people who want an increase in buttock volume and who have enough fat tissue in their bodies for fat transfer.

It is important to be in good general health, to have a relatively stable body weight and to be able to follow the care rules recommended after surgery.

People with a very low body fat percentage may not have enough fat tissue for transfer. In patients with marked skin sagging, fat transfer alone may not provide the desired result.

Uncontrolled diabetes, serious heart or lung diseases, clotting disorders and other important health problems can increase surgical risks.

Suitability for surgery is determined after an assessment by a specialist in plastic, reconstructive and aesthetic surgery.

Risks and Complications of BBL Surgery

Unlike other aesthetic surgical procedures, BBL is an operation that carries significant risks, especially in terms of fat embolism.

Fat embolism is a serious complication that can occur when the transferred fat enters the vascular system and can be life-threatening. For this reason, it is of great importance that the injection is not made into muscle tissue and that the safe anatomical layer is preserved.

Bleeding and infection are other complications that can be seen after surgery.

Seroma is a collection of fluid in areas where fat was removed or surgical intervention was performed. Additional intervention may be needed in some patients.

Fat necrosis can occur when some of the transferred fat cells lose their viability. This can cause firmness, nodules or contour irregularities.

Asymmetry and volume loss can occur because the fat cells take in different proportions.

Deep vein thrombosis and pulmonary embolism are among the serious complications that can develop because of immobility after surgery and the surgery itself.

The development of symptoms such as sudden shortness of breath, chest pain, fainting or changes in consciousness after BBL requires emergency medical intervention.

Recovery After BBL

After BBL surgery, swelling, bruising, tenderness and pain can be seen both in the areas where fat was harvested and in the buttocks.

Resting in the first days is important. However, starting controlled walks at the time the doctor considers appropriate can help support blood circulation and reduce the risk of clot formation. Long periods of bed rest are generally not recommended.

Sitting and Sleeping Position

It is important not to apply direct pressure to the buttock area where the fat transfer was performed.

After surgery, avoiding sitting directly on the buttocks for a certain period or using special support pillows may be recommended.

The sleeping position should also be planned according to the areas where the transferred fat is located. In some patients, lying face down or on the side may be recommended.

The duration of these restrictions is determined according to the technique applied by the surgeon and the patient’s recovery.

Wearing a Compression Garment

Special compression garments may need to be worn on the areas where liposuction was performed.

These garments can help control swelling and support the areas where fat was removed.

However, it is important not to apply excessive pressure to the transferred fat tissue when using a corset.

Returning to Daily Life

Many patients can return to light daily activities within the first few weeks. However, a longer wait may be needed for jobs that require sitting for long periods and for intense physical activity.

The return to heavy exercise is generally carried out a few weeks later, with the surgeon’s approval.

While most of the swelling decreases over time, it can take several months for the final appearance of the buttocks to emerge.

Different Techniques and Alternatives to BBL

The Brazilian butt lift is basically the transfer of the person’s own fat tissue to the buttocks. However, there are also other surgical methods to increase buttock volume or correct sagging.

Buttock Implants

In buttock implant surgery, volume is increased using silicone prostheses.

This method can be considered especially in people who do not have enough fat tissue for fat transfer.

However, implant surgery has its own particular risks, such as infection, displacement of the prosthesis, capsule formation and wound healing problems.

Combining Implants and Fat Transfer

In some patients, buttock implants and fat transfer can be planned together.

In this method, volume is increased with the implant while contour irregularities in certain areas can be corrected with fat transfer.

However, combined surgeries can require more extensive intervention and carry additional risks.

Surgical Buttock Lift

In people with marked skin sagging in the buttock area, a surgical buttock lift can be considered.

In this method, excess skin is removed and the aim is to reposition the tissues upward.

Unlike fat transfer, a surgical buttock lift aims primarily to correct sagging rather than to increase volume.

When Do BBL Results Become Clear?

An increase in buttock volume can be seen right after BBL surgery. However, part of the initial appearance is due to the swelling after surgery.

Not all of the transferred fat cells stay viable in the new area. Some are absorbed by the body over time, and a certain amount of decrease in buttock volume can be seen.

The results generally become more apparent within 3–6 months. In some patients it can take longer for the tissues to settle completely.

Fat cells that stay viable can provide a long-lasting increase in volume. However, marked weight gain, weight loss and the aging process can affect the results.

Keeping body weight stable helps maintain the contour achieved.

Common Misconceptions About BBL

One of the most common misconceptions about BBL is that the procedure is completely risk-free. In fact, fat transfer to the buttocks is a surgery that requires serious safety measures, especially because of fat embolism.

Another misconception is that all of the transferred fat is permanent. Because some of the fat cells are absorbed by the body, the volume right after surgery may not be the same as the final result.

The idea that BBL will provide the same look in every patient is also not correct. Results vary according to the patient’s anatomical features, the amount of existing fat, skin elasticity and the surgical technique used.

The idea that BBL consists only of adding fat to the buttocks is also incomplete. The aesthetic result of the procedure is evaluated together with the shaping of the areas where fat is removed. However, body contour goals should never take precedence over surgical safety.

Conclusion

The Brazilian butt lift (BBL) is an aesthetic surgical procedure that aims to increase volume and change the body contour by transferring fat tissue taken from the person’s own body to the buttock area.

BBL can offer long-lasting results in suitable patients. However, because there are serious complication risks, fat embolism above all, it is of great importance that the surgery is performed with safe techniques.

Before deciding on the procedure, general health, the existing fat tissue, aesthetic expectations and surgical risks should be evaluated in detail by an experienced plastic surgeon.
